The biggest challenge I encounter when applying 2.0 or 2.1 is when using open source software combined with custom programming. If the OSS is not accessible and they're not willing to fix it in the main branch, you get stuck with your own version that gets out of sync.

Another challenge is finding developers and testers who know what they're doing and don't see accessibility as some test you do when it's done.
