So there really is no way to make Node.js add and send HTTP header names in their mixed case form, as God intended them to be sent?

Can you be more specific with what you've tried? Sending mixed-case HTTP headers works fine.

For purely aesthetic reasons, I’m unhappy with “content-type” and can’t make Node/Express send Content-type instead

Just checked, both res.setHeader("cOnteNt-tYpe",...) & res.writeHead(200, {"cOnteNt-tYpe": ...}) work for me (plain vanilla node)
