“Federal agency warned about risk” was not enough to prevent this girl’s death; the agency should have been able to demand the necessary change. Safety should always always always come first.

Safety should always be first but when it comes to big money $$$ rule. Like Remington continuing to sell rifles for decades knowing they had a flaw which allowed firing without the trigger being pulled. Killed dozens & injured 100s. Everyone looked the other way.
