I run my own Minecraft 1.12.2 server and it seems our server has been Blacklisted by Mojang.
Is there an appeals process to have our server removed from the blacklist? We've removed the offending modification and any associated folders with it.

Minecraft Support can't comment on any cases regarding account suspensions or bans. If you've been suspended from your account, please submit a ticket via the "Case Review" form at the bottom of our help site: help.minecraft.net/hc/en-us. ^BT

I've not had my account banned, or suspended. We had our Minecraft server Blacklisted. I've checked that link already, it doesn't mention anything about server blacklist appeals.

Yes, it's a Java server. 1.12.2 specifically.
What options do I pick for my issue? As mentioned, I've been on that site, and the drop down menus don't appear to have my problem listed.

Fill out the needed options then click Account Support and you should be good to go. Make sure you fill in the description with everything needed and our Support should be able to help! ^BT
