We can help you with this but it appears they asked follow up questions we need you to respond to in the ticket. Issues of this nature do need to be handled in the ticket itself. ^AC
Sorry for the confusion here. The ticket number should have been created when you first submitted the initial ticket. It would have said request # then had a 7 digit number. Are you able to find that initial request you sent? ^AC

how odd. Could you try submitting that request again through the link I provided? That should allow your ticket to go through and you can grab that ticket number so we can make sure it gets seen for you. ^AV

Looks as though its asking you to fill out the form needed to submit that ticket once you've filled that out and submitted that ticket an agent will get back to you as soon as they can to help ^AC
