Might be a silly question, but is your portal inside a Nether fortress? Wither skeletons can spawn anywhere in a Nether fortress if it's dark, even if it's next to your portal. ^IV

Hmm, this usually means that there's an issue with the game files that's preventing new worlds from being created. Reinstalling the game should fix it. Be sure to back up your existing worlds first, otherwise they'll be erased. See our guide here:
