When you're seeing demo mode it usually means you're logging into an account with no purchase of the game. I would doubel check the gamertag/username and make sure you're signing into the Microsoft login. ^TK
Okay, so the game is frree to play through PS Plus or you downloaded the game in free to play and it's not downloading? ^TK

Sometimes subscriptions can break in Realms! Did you happen to extend the realm or were you trying to create a new one? ^TK
Once a Microsoft account has been linked to a PSN account, neither of the accounts can be associated with a different account. Unlinking accounts will cause your purchases to be returned to the original Microsoft account and will no longer be available on the PSN account. ^TK
