I kind of had this happen once on PS3. Boone died but his textures stayed in the air t-posing. I poked him and I guess his gibs were frozen inside his skin textures, because he then appeared to just shit his entire innards.

I know this is satire but the really easy solution here would be to require everyone running glitchless to install YUP, NVTF, etc. and the game would run flawlessly besides a few engine-level bugs. Source: Published FNV and TTW mods for years.
